Bioidentical Hormone Therapy vs Synthetic — What's the Difference?
If you are exploring hormone replacement therapy, you have probably encountered the term bioidentical hormones — and you may have gotten conflicting information about what they are, whether they are safer, and whether they are right for you.
What Does Bioidentical Mean?
Bioidentical hormones are chemically identical in structure to the hormones naturally produced by your body. The term refers to molecular structure — not whether a hormone is natural or synthetic in origin. Bioidentical hormones are typically derived from plant sources and processed in a lab to match your body's own hormones exactly.
Synthetic hormones have a different molecular structure. The progestins used in many conventional HRT formulations — like medroxyprogesterone acetate, used in the Women's Health Initiative study — are not the same molecule as the progesterone your body produces.

Why Does Molecular Structure Matter?
Your hormone receptors are designed to receive specific molecules. When a hormone fits the receptor precisely, the biological response closely mirrors what your body would produce naturally. Synthetic hormones that do not match the natural structure can activate receptors differently — which accounts for differences in side effect profiles.
This distinction became particularly important after the Women's Health Initiative study in 2002. It is now widely recognized that the synthetic progestin used in that study behaves very differently in the body than bioidentical progesterone.
FDA-Approved Bioidentical Options
It is a common misconception that bioidentical hormones are not FDA-approved. Several FDA-approved medications contain bioidentical hormones, including estradiol patches, gels, and sprays, as well as oral micronized progesterone (Prometrium).
Compounded bioidentical hormones are custom-formulated by compounding pharmacies and allow for highly individualized dosing not available in standardized commercial products.
Is Bioidentical HRT Safer Than Synthetic?
The research is still evolving, but current evidence supports:
- Bioidentical progesterone appears to carry a more favorable breast safety profile compared to synthetic progestins, based on observational studies including the large French E3N cohort study.
- Transdermal estradiol appears to carry lower clotting and stroke risk compared to oral estrogen, because it bypasses first-pass liver metabolism.
- Risks vary significantly based on age, timing of initiation, type of hormone, dose, and route of administration.
The current consensus among menopause specialists — including the Menopause Society — is that HRT is appropriate and beneficial for many women, particularly those who start within 10 years of menopause or before age 60.
